New Patients

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

Eligibility can be quickly confirmed from your address so it would help if you could provide proof of address if you have this but if you do not, we may still be able to register you.

You will need to complete a registration form (GMS1) and a health questionnaire (both forms also available below or use the online registration forms) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Our reception team can also help you register with us. Please call them 10am and 5pm, if you need help to register. It maybe helpful if you can bring:

  • The name and address of your previous NHS GP, or the date you have entered the country if you don’t have a previous GP.
  • Details of your current and previous address.
  • Photo ID and proof of address if you have these, but not necessary. If you don’t, we can still register you but it may take significantly longer to trace your previous notes where this is required.
  • When registering infants please bring their red book, if able.

If you do not speak English or find it hard to complete forms for any reason please let us know so that we can help with the paperwork.

By registering with the practice, you are granting permission to the practice to contact you using the details provided unless you state otherwise. Privacy Policy available on website under your health records and  information sharing section.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. Please note, you may be asked to provide a urine sample.

Medical treatment is available from the date of registration. Please contact reception for further information.

Your named Accountable GP will be Dr Chawdhery and she will be responsible for overall care at the practice.
